Transition Recovery After Cancer Experience Clinic... |
As
cancer therapy evolves, an increasing number of patients are being
cured and becoming long-term survivors of childhood cancer. The
design of the Transition Recovery After Cancer Experience (TRACE)
Clinic is to address the concerns and needs of our survivors. Our
goal is to assess patients’ medical and psychosocial needs, provide
information on such subjects as life after treatment, life-long
medical care, healthy living behaviors and referrals to other
services as needed. Patients and parents will meet with a group of
our staff dedicated to the care of long term survivors. This group
includes oncologists, nurses, social workers, psychologists,
nutritionists, and school support personnel who will assess the
individual’s needs and make recommendations for follow up. We will
provide families’ a detailed letter with the therapies their child
received. A comprehensive letter will be sent to the child’s
primary care provider detailing the therapies the child received,
assessments from staff as well as a problem list/plan of action
based on the assessments. Patients and families will be provided
with educational materials specific to their needs and potential
risks as well as a survival guide and book on childhood cancer
survivors.
This program is offered at the
Children's Cancer and Blood Disorders Clinic at Palmetto Health
Richland.
